Reliability, Risk & Safety Engineering

Pentagons' Reliability Engineers can undertake Safety and Reliability, Availability, & Maintainability (RAM) studies to assist in the identification and management of safety and asset risks. We can provide specialist advice in the management and mitigation of Safety and RAM risk and can undertake the following types of analysis to identify and mitigate risk.

Forensic Failure Analysis & Investigation

Pentagon Consulting Certified Materials Engineers are ASM International qualified Failure Analysts and Investigators and can undertake Forensic Engineering Investigations and Failure Analysis to determine a system or a component's failure mode. The end goal in failure investigations is to determine the root cause and recommend a solution which would prevent future failure events.

PHA - Preliminary Hazard Analysis

The preliminary hazard analysis (PHA) technique is a broad, initial study used in the early stages of system design. It focuses on identifying hazards, assessing the potential risks, and identifying risk mitigation strategies.

FMEA - Failure Mode and Effects Analysis
An FMEA involves reviewing a systems components, assemblies, and subsystems to identify failure modes, and their causes and effects. FMEA is an inductive reasoning single point of failure analysis.

CA - Criticality Analysis
A Criticality Analysis, undertaken in conjunction with an FMEA, can be a qualitative analysis but may also be undertaken on a quantitative basis when reliability prediction data or mathematical failure rate models are developed.

Weibull Distribution
The Weibull Distribution is exceptionally useful for predicting time to event analysis. A primary advantage is the ability to provide reasonably accurate failure analysis and failure forecasts with extremely small samples.

Poisson Analysis 
The Poisson Analysis is used to determine the probability of an event occurring within a given time or space, e.g. the number of failures or accidents in a given time or defects in a length of a turbine shaft.

FTA - Fault Tree Analysis
FTA is a top-down deductive failure analysis whereby the undesired system state is analysed using Boolean logic to define and link a series of lower level events.

ETA - Event Tree Analysis
ETA is bottom up, logical modelling technique which enables the creation of success and failure pathways emanating from a single event. It allows determination of the pathway creating the greatest probability of failure for a system.

For work requiring Security Clearance, Pentagon has AGSVA Security Clearance to NVL1 and can obtain NVL2 if required
